It is important to let us know if you are a carer.
A Carer by definition is someone who, without payment, provides help and support to a partner, relative, friend or neighbour, who could not manage to stay at home without help due to age, sickness, or disability.
We fully appreciate that being a Carer can at times be very tiring. We hope that by identifying all our carer’s registered at the practice, we will be aware of your circumstances and assist in your health care. We will make a note on your records to highlight you are a carer, offer you an annual appointment to ensure you are doing ok and provide you with a copy of our Carer’s Information Pack. This includes useful contact numbers, services that are offered at the practice and information leaflets. Please speak to one of our Reception team and they will provide you with the form and offer you the relevant information.
Are you a young carer
If you are looking after or regularly helping someone else whether that is a family member or someone else and you are aged 8 – 18 years of age, we can provide you with information regarding the Young Carer’s Project. Please speak to one our Reception team and they can provide you with the information you need.
